UNIOSUN Freshers' Documentation: Items to Bring

I have previously discussed general requirements for school physical screenings or freshers' documentation in my post, "Documents Candidates Need For Freshers' Screening/Registration". In that post, I wasn't specific about a school, as it addressed all candidates who would be processing admissions into any school adopting the physical screening method for its admission. The same set of schools will ask for credentials physically when you resume school for freshers' documentation.


In this post, I will be specific about Osun State University. The university has campuses across the state. However, irrespective of your own college, once offered admission, you pay a few fees, the next thing is to appear on campus for the freshers' document/screening.


This screening will require you to bring along certain documents. Some will be generated from the school admission portal, while a few others will be sought by candidates themselves.


To save you the headache, since UNIOSUN has been consistent with the same credentials and items, over the years, this post will list what you need to take along and give brief information to guide on how to get them.

List of Items Required for UNIOSUN Freshers' Screening

  1. UNIOSUN Screening form
  2. UNIOSUN Provisional Admission Slip
  3. Two copies of UNIOSUN Acceptance Clearance
  4. UNIOSUN Medical form
  5. JAMB Admission Letter
  6. Original JAMB result
  7. Original and Photocopy of O’Level Result
  8. O'level result checking card
  9. Statement of Result and Copy of Academic Transcript (Direct Entry Only)
  10. UTME Result/DE Registration Form
  11. Post UTME Screening Result
  12. Valid (three years) Tax Clearance of Parent
  13. Osun State Development Levy
  14. Birth Certificate or Declaration of Age
  15. Letter of attestation from a reputable personality
  16. Passport Photos


As earlier mentioned, there are a few things to know about each of the items above. Let's dive into it.


  1. UNIOSUN Screening form: This was the form you printed out after filling and submitting the UNIOSUN screening form online. If you registered at a cafe back then, this two-page printout was supposed to be given to you. If you registered yourself, you should have printed it too. If not or misplaced, just log in to your UNIOSUN admission portal and click on "Print Application Forms" A pop-up will load the two-page form for you to reprint.
  2. UNIOSUN Provisional Admission Slip: After being offered admission on the school portal, you're to pay about N3000, including the remita charges. After paying this, you'll be able to see and print the "Provisional Admission Slip." If you don't have that printed yet, log in to your portal and print.
  3. UNIOSUN Acceptance Clearance: Access to the "Provisional Admission Slip" only occurs after paying the admission access fee, as stated above. You'll not have access to the Acceptance Clearance Slip until you pay for the acceptance fee. It's called "Acceptance Clearance Letter" right on your admission portal. After paying the acceptance fee (which is about N41,000 at the time of this writing), you'll also need to fill a basic form before this letter will be prompted for printing. The basic form is better called the acceptance form.
  4. UNIOSUN Medical form: After paying for the admission access fee, the portal will open acceptance fee payment. It's only after paying that the portal will open the medical fee page too. The Medical form is usually a two-page document that you'll be prompted to print right after paying for the medical (which is about N21,000 at the time of this writing). This will be filled after payment with a few biodata. Then, after printing, you're not to fill anything, on the paper, until you get to the school clinic where the medical test will be conducted and right spaces on the printout will be filled by the medical personnel.
  5. JAMB Admission Letter: JAMB admission letter will be printed from the JAMB portal. This will not be available until you're offered admission on the JAMB CAPS, accept it and pay for it. It's a two-page document containing a copy for the candidate and the other copy for the school, as indicated on each. Whereas, for the purpose of the freshers' screening/documentation, you're to present the institution copy at the point of the checking while you keep the candidate's copy for your own use.
  6. Original JAMB Result: This is available to all JAMB candidates almost immediately after the board releases the UTME results. If you've not printed this before, log in to your JAMB portal, pay for and print the UTME result. If you've printed it before but you later changed school and/or course, you'd need to reprint so that the new changes will reflect on it before going for your fresher's documentation.
  7. Original Copy O’Level Result: O'level result is your SSCE result, (or simplify your WAEC, NECO, or NABTEB result) depending on the one used for your screening and UTME. If you use two sittings, you'll have to go with the two results. If your result is a year old or more, your school should have the original copy ready for pick up. If the result is less than a year, you may not be able to get the original one yet. In this case, the screening officials are aware and will accept the online printout of your result. Of course, you may need to reprint it out in color, if you can. However, if you can't afford in color, it's still acceptable.
  8. O'level Result Scratched Card(s): The screening officials will want to check your O'level result by themselves. Apart from giving them the printout, they want you to buy a new WAEC, NECO, or NABTEB scratched card which they will, themselves, use to check the result. Even if you have an existing card with you, they may not accept that, they want a new one. So, buy another not-yet-scratched card. Moreover, at the venue of the screening, the officials are selling cards for a price of about N4,000 at the time of this writing. You may be forced to buy theirs notwithstanding. If you used two sittings, you'd buy two cards!
  9. Statement of Result and Copy of Academic Transcript (Direct Entry Only): Direct entry applicants are those who have completed IJMB, JUPEB, NCE, ND, RN, HND, BSC, etc. before applying to further their studies through 200-level of a program at the university. These candidates are different from UTME applicants. This section strictly affects the former. You're to go for the documentation with the statement of result from your former school or qualification together with a copy of your transcript. You may not worry about the transcript if you've earlier requested that to be sent to the UNIOSUN registrar, as instructed during the online screening registration. And if your school won't give you the transcript by hand, you may use my tricks in "How To Collect Transcript By Hands/Post to Yourself" to get it delivered to you.
  10. UTME/DE Registration Form: This might be asked for or ignored. You should, notwithstanding, go with your JAMB registration slip. Of course, there is no need for a reprint if you've chosen UNIOSUN as your first choice from the start. However, if you changed institution or course later, you might need the reprint. The reprint could be the one given to you by the CBT attendant, after the changes were made. You can also check the email used for the JAMB (linked to the JAMB portal). A copy of the early and final registration/changes slip will usually be available to download and print from there.
  11. Post UTME Screening Result: Of course, UNIOSUN doesn't conduct post UTME; rather, it uses screening. After registering online, a result will be made available to you a few days or weeks before admission lists will be out. This is the screening result in this context. If you've not printed it, log in to your UNIOSUN admission portal, under the "RESULT STATUS" you should see "AVAILABLE [ PRINT RESULT SLIP]". Click on that to print "SCREENING EXAMINATION RESULT SLIP".
  12. Parents' Tax Clearance: This document is better obtained within Osun State. This is because it will include the Osun State Development Levy. It's the tax clearance to be collected by your parents or one of them. At the time of this writing, it's N12,000. It's available at literally any local government area within Osun State.
  13. Birth Certificate or Declaration of Age: Among the personal credentials you're to submit for documentation is the birth certificate or the age declaration. While a birth certificate can be obtained at the hospital of birth or national population commission (NPC) of any state or local government, you can get the age declaration at any customary, magistrate, or high court. For a price of about N2,000, you'll get a copy of birth certificate or age declaration if you don't have such before.
  14. Local Government Identification: This is also called the state identification, state of origin certificate, or certificate of citizenship or indigeneship. It won't matter the name they call it around you; it means the same thing and serves the same purpose. This local government identification must be the same as the one on your JAMB portal and screening portal. In case you're not sure, check your JAMB registration slip, UTME result or change of institution slip for the local government on it. You may get this for about N5,000 depending on the local government area.
  15. Letter of attestation from a reputable personality: You're to go with the letter of attestation. This is also called a letter of guarantor. It should be written by a pastor, imam, chief, public officer, or proprietor/principal of your former school. The letter is better written and typed on a letterhead showing the position/organization of that person.
  16. Passport Photo: About four copies of your passport photos will be asked for during the documentation for certain official uses. It's not stipulated whether the passport photos should be a background, but I recommend you get white or red background.

Color Printing and Photocopies of All Documents Is a Must

So far so good, we've covered the documents/credentials you will need for the UNIOSUN freshers' documentation. However, all the documents must be in color and must be original. Though black is acceptable if you can't get color.


As a result, the officials won't take down all the original items. They'll have their photocopies with them while returning the original ones. This is why you should have at least three photocopies of each of the items. These photocopies will be needed at the center.
