Though you may want to try registering the application by yourself, it's nearly impossible to do that, because you'll need to use a fingerprint scanner, at a point, during the registration.
Hence, it's more like a waste of time or an invitation to stress to start the registration yourself or make an attempt to complete it on your own.
Resolved to use the service of a nearby approved cafe for the NYSC mobilization registration or do you still want to handle things yourself? The rest of this post is the list of information and items that will be filled during the registration.
Note that, whether you'll be posted (as a full-time graduate who is within the serving age), or you won't be posted (as a part-time or over-aged graduate), you will be required to supply the same information during your NYSC registration.

List of Information to Filled While Registering For NYSC Mobilization
- Names: You won't need to fill in names during the NYSC registration. Your names had already been sent to NYSC through the senate list. And if for any reason, you must correct the names sent, you can follow the guide in, "How to Do NYSC Change of Names, Date of Birth and Course of Study".
- Date of Birth: You won't need to fill in the date of birth too, as this will be imported through your JAMB registration number.
- Home Address: This is the address of the residence/contact or that of origin as necessary. Married women, especially, should be sure this address aligns with the addresses of their husbands as stated on the husbands' local government identifications or the utility bills for the husbands' states of residence.
- Local Government of Origin: You will fill this out yourself. However, your state of origin must have been exported from your JAMB registration number too. Hence, you can't change the state. And you won't need the physical paper from the local government of origin during your registration.
- State of Origin: You can't change this during the registration which will force you to choose the local government you claimed during the JAMB registration too.
- Sex and Marital Status: You will fill this out during the registration. Taking the single will automatically force the system to identify you as Miss. or Mr. Married women, who take Married, will automatically be identified by Mrs. and they will be required to fill in their husbands' full names. Married men will remain Mr.
- Health Status: You will be asked to take between Good and Poor. If you take the Poor, the system will ask you to indicate your health condition and you will be required to upload a health/medical report before you can submit your mobilization form. Here, even people with poor health will still take the Good but will use Medical reports at their NYSC camp to redeploy. However, if you want to be posted directly to a specific state and you have the Medical Report report ready, you can put in poor and follow other instructions. - Blood Group and Genotype: You will be required to fill this out during the registration. It may be wise to do these tests before you fill out the online form. Some prospective corpers know this from their 100-level medical test or any other. However, if you just pick one at this point, be sure you repeat the same choices in your medical fitness certificate.
- Matric and JAMB Registration Number: You will need these while creating your NYSC profile. These are the details that will help the system identify and import your information from the JAMB portal and the senate list. After supplying these two numbers correctly, at the first stage of the registration, the system should display your Full name, course of study, date of graduation, and grade as sent by your school and JAMB.
- Next of Kin Details: You will be required to fill in his/her name, address, relationship with you, and phone number.
- Person In case of Emergency: You will be required to fill in his/her name, address, relationship with you, and phone number.
- Person In Case of Death: You will be required to fill in his/her name, address, relationship with you, and phone number. Note that, you can't repeat the same person as your "next of kin" at the same time as "in the case of an emergency". But you can use any of the two in the case of death.
- Primary School Name and Years of Attendance: You will be required to fill in the name of your primary school, and the location together with starting and the ending year. The day and months don't matter if you don't know them. If you attend two or more primary schools, take one and calculate the 6 years for that one. It's just formality!
- Secondary School Name and Years of Attendance: You will be required to fill in the name of your secondary school and the location together with starting and the ending year. The day and months don't matter if you don't know them. You won't need to separate JSS from SSS. If you attend two or more secondary schools, take one (one of the two, in case of two sittings) and calculate the 6 years for one. It's just formality! Also, you may want to use the name of the school where you got your SSCE result. Where it's GCE, use the name of the secondary school you originally went to.
- Higher Institutions' Names and Years of Attendance: You will be required to fill in the name of your university, polytechnic, or college and the location together with starting and the ending year. The day and months don't matter if you don't know them. HND holders are to fill their ND information under "Foundation" and HND information under the "Tertiary" heading. University graduates who gained admissions through UTME will fill for tertiary institutions only. Those who used the direct entry will fill their institutions for NCE, ND, JUPEB, IJMB, etc. under the "Foundation"
- Signature: You will be required to sign on white or plain paper. Then, scan and upload the signature
- Passport Photo: If you have a white background passport photo, take it along to the café. However, a professional and approved café should make provisions for taking the right passport with the right specifications.
- Other Information: Other information, while filling NYSC mobilization form, include the national awards (if you have any), the size of your shoes, shirts, and trousers, the local languages you speak, and the list of states you've visited before

In the end, the NYSC mobilization form is truly a complex form that has about three stages for single graduates and one more stage for married women who will still be required to scan and upload all their supporting documents.
Obviously, most of the details here can be remembered if you visit the café without any paper. However, if you will have to track your brain to recollect anything at the point of the registration, it may be wise to write the information out on a paper before visiting the nearest approved café for your NYSC mobilization registration.
