How to Reprint Regularization Slip/Indemnity Form

After the JAMB regularization application, you're to print a form called the indemnity form or slip. Some may call it a regularization slip. This will be taken to your school for signing and forwarding to the nearest JAMB office before the approval can be given by the board.


However, there may be a need to get this slip back if lost in transit. Your school might have misplaced or lost the indemnity slip/form. You might have lost it yourself before getting to school. 


In fact, there may be a need to resend the indemnity form if there is any discrepancy between JAMB and the school during the approval process.


So, how do you get this indemnity or regularization slip/form back? Can you get the printout back without reregistering the regularization? Is it possible to print it through your JAMB profile?

Return to The JAMB Office or CBT Centre Used to Get The Indemnity Form/Slip

To register for the regularization, you must have used the service of a particular JAMB office or CBT centre. As long as you can still remember your regularization profile (code) email and password, the office/centre should be able to log in to your account and reprint the slip for you.


However, it's not compulsory that you use the service of the same JAMB office or CBT centre that registered you. You can walk into any JAMB office or approved CBT centre to table your need. They have direct access to your profile with email and password or your profile code.

Log In To Your Regularization Profile and Print Yourself

If you have a connected computer or mobile device, you can log in to your JAMB profile and reprint the indemnity form/slip. And if this may be difficult, you can use the service of a nearby café or internet business centre.

Below is the procedure to print/reprint the JAMB regularization indemnity form, slip or printout.


  1. Visit the official JAMB e-facility page or click https://efacility.jamb.gov.ng/Login
  2. Enter the email used for the regularization together with the password taken during the registration
  3. Click on "Login"
  4. If things are correctly entered, this will take you to your JAMB dashboard
  5. Here, you may need to use a laptop instead of a mobile phone. A desktop view will let you see all the content of the dashboard including the next link you will click on.
  6. Meanwhile, if you know how to change a mobile view (on your phone browser) to a desktop view, you should be fine.
  7. In the desktop view, right towards the bottom of the dashboard, and right above the Log Out link, by the left of your dashboard, locate "My Indemnity Forms".
  8. Click on this to access the link to your regularization slip. Click the link to download the PDF file and send it to the printer.

  2. Pleaplease help me i did my jamb regularatition on 28 april and 2021 and this year 27 February i was sent to print my condonement of illegitimate admission i did and i paid but uploading the year of admission is the problem

  3. Good day Sir, I submitted my indemnity form in my school and after a long while say 4months nothing is happening so I went to the school to check and they wrote "unable to send email to specify address" on my indemnity form. Please Sir, what does it mean and how can I go about it

    1. Maybe the email you used for the regularization was not correctly or not existing before using it. However, you can use any the methods detailed in the post below to check if the regularization is already successful.

  6. Hi, so in my Birth Certificate and University details I used 1998 as DOB, but unfortunately during my jamb registration 2018 I had used 1996. Now during regularization this year, I had to change the DOB of the jamb from 1996 to 1998 to tally with that of my current university details. However my WAEC still remains 1996. I have done this regularization ever since March but until now no message from jamb yet..I did check my jamb Profile (indemnity form and it says:

    1. Application not yet approved @HQ

    2. Candidate Application not yet uploaded @State Office

    3. Form upload authorized by institution

    My question is, could it be possible they have refused to upload it at jamb office because of those errors/corrections I made?

    1. No. It's not because of those errors. They may just be delaying things unnecessarily. You should send a compliant to JAMB through the link below.

Good day Sir, I transferred redeemer University to Joseph ayo baba Lola University, so I went to jamb office to do change of institution since am no longer in redeemer, I did it successfully and I was given an indemnity for to be given to my former institution and current institution to sign so it can be processed, I am yet to go to my former institution (redeemers) for dem to sign by the registrar because I also have a job, I just want to know if when my admission status will change from redeemer to jabu, if whether because I haven't gone for d signing it is delaying it and what when happen next if both institutions are done signing what happens next?

I was shortlist amongs nysc last year but when I went to register my information was not there I was told I have issues with my admission letter of which I have been processing since 2014 till date not successful I have submitted two indemnity form of which till now it's not processed I have been to my school more than 8 times still no results
